This electronic feedback form provides the opportunity to send questions and comments to the NHS Academic Structure Working Group.  This can be done anonymously (in which case you should not enter your e-mail address) or not, at the user’s discretion.  Please note, however, that there is no mechanism for the Working Group to respond electronically to items that are sent anonymously.
